The RAID Log Nobody Actually Updates (And How to Fix It)

Almost every project starts with a RAID log. Almost every RAID log stops being updated by week three. The problem usually isn't the format - it's that the log lives somewhere disconnected from the meetings where risk actually gets discussed.
The fix that has worked consistently: review the top three open items live in every weekly sync, out loud, with an owner and a next step assigned on the spot. A RAID log that's discussed weekly stays alive. One that's only reviewed during a monthly steering committee is already dead.
